Blown Nozzle and Outter Shield

good Afternoon everyone,
I have a Messer MetalMaster+ with 2 HPR400XDs. The other day I was cutting 5 x 5 squares with a 1.5" hole in the center out of .5" A36 steel plate with a 130a. All of a sudden one of the torch's consumables blew out after approximately 12 pierces. Looking at the consumables the outer shield and nozzle have a tear drop shaped blow out. The electrode, swirl ring and water tube were all in pristine condition. I changed the consumables and the same happened after 12 or so pierces. looking at the diagnostics the plasma and shield pressures seem to be right on. The height also seems to be correct. Anyone have any advice or ideas?

What gas/gases are you using?
