Double pierce?

What would cause this nutsack pierce? This is a test cut. We dropped delay to 0, pierce height dropped to .075, 100 volts with standard nozzle setup. Tried all kinds of settings but keep getting this. Rest of the cut looks decent

Giving some info about your table, plasma unit, and software would help. The picture looks like that is stainless. I would say you have some movement at the time of pierce, unless you actually see the torch pierce twice, which make it likely it is a g-code problem. when cutting an open loop or line there is no lead in so the cut should start on the line with no sideway movements. The pierce time is just to allow enough time to penetrate the metal, to short no penetration, to long a bigger hole, so no sideward moving unless slop in the table.
Changing the voltage only affect the cut height after the torch starts moving and the THC is in control, is not used at pierce time.

Wright CNC 4x4 table Hypertherm 45XP using Sheetcam. Material is 16 gauge mild steel.
