I hope it was a Hypertherm PowerMAX85 on a Dynatorch CNC Plasma Table . . . .

I don't know myself. I'm wondering what kind of machine can offer that kind of cut quality. Note the long holes have hardly any bevel. There is some beveling as you can see on the sharp corners where the other smaller holes meet the edge of the part. Not sure if water or laser would make that kind of cut.
I'm pretty certain it isn't air plasma unfortunately.

If you told us the type and thickness of material...and if the photo was in focus I would dare to take a guess. The approximate size of the part would help too.
If it is 3/8" thick steel and the slots are at least 3/8" wide you could get a part like that cut with a precision cnc plasma equipped with a Hypertherm HPR130 equipped with True Hole technology.
The lag lines appear like a thermal process (not abrasive water jet) so it likely is plasma, oxy-fuel or laser.
